NEPHU Population Health Catchment Plan. Part 1: 2023-2025

The North Eastern Public Health Unit (NEPHU) is a vital component of Victoria’s Public Health Network. Our purpose is to work collaboratively with a range of stakeholders to deliver a comprehensive public health program for the region. To support this role we have developed a comprehensive Population Health Catchment Plan, Evaluation Strategy, and Evaluation Toolkit—designed to guide effective health interventions and enhance community well-being across the NEPHU catchment.

NEPHU Population Health Catchment Plan Part 1 (2023-2025)

The NEPHU Population Health Catchment Plan aims to guide collaborative efforts and prioritise health promotion and prevention initiatives to support communities in living healthier lives – now and into the future. 

Our Population Health Catchment Plan sets out the priorities and potential actions for prevention and health promotion over the next two years from 2023-2025, and articulates: 

  • A framework, including principles and approaches, to guide how NEPHU, partners and stakeholders work together to align and prioritse health and wellbeing outcomes for the community.
  • A population profile including demographic, environmental and burden of disease information for the catchment 
  • A model for collaborative and collective action that enhances coordination of effort.

Developed in partnership with key stakeholders and informed by a multi-stage planning process – which included a review of the prevention landscape and consultation with over 40 stakeholder groups, – the plan identifies two initial priority areas for collective action: 

  • Increasing healthy eating 
  • Improving sexual and reproductive health. 

These health priorities are directly connected to preventing avoidable chronic diseases such as heart disease, type 2 diabetes and cancer. 

The development of this plan is guided by the principles of collaboration, equity, and empowerment, and informed by various approaches – such as collective impact, evidence-informed strategies, and intersectionality. 

NEPHU Population Health Catchment Plan Evaluation Strategy 2023-2025

The Evaluation Strategy was developed with the NEPHU Population Health Catchment Plan Steering Group and Priority Area Program Control Groups.

The Strategy is comprised of three principal components:

  • A framework for evaluating the overarching NEPHU Population Health Catchment Plan
  • A generic framework for evaluating Priority Area Action Plans, which is accompanied by a supporting toolkit
  • A guide for the evaluation of individual initiatives within Priority Area Action Plans

Evaluation strategy toolkit

The Evaluation Strategy is supported by NEPHU Catchment Plan Evaluation Toolkit. The toolkit contains a resource list with links to a range of high-quality resources that contain broader evaluation information, examples and ideas.

Population Health Catchment Planning Reports

The Population Health Catchment Planning Reports outline the process undertaken in determining priorities for place-based primary and secondary prevention activity focused on preventable chronic disease and modifiable risk factors.
