Population Health Catchment Plan Part 2: 2025-2029

The NEPHU Population Health Catchment Plan 2025 to 2029 sets the direction for NEPHU’s work in prevention and  health promotion over the next four years. 

The NEPHU Population Health Catchment Plan – Part 2 (2025 to 2029) outlines the key health and wellbeing priorities that the Population Health Branch will prioritise, develop and lead in partnership with our stakeholders and communities. 

The goal for this work is to improve health outcomes and reduce health inequities across the region. 

Part 1 of NEPHU’s inaugural Catchment Plan was delivered from 2023 to 2025, focusing on the two priority areas of improving sexual and reproductive health and increasing healthy eating. 

Building on learnings from part 1, extensive consultation and data analysis, Part 2 expands NEPHU’s focus to five priority areas for action from 2025 to 2029: 

  • Improving sexual and reproductive health 
  • Reducing harm from tobacco and e-cigarette use 
  • Improving wellbeing 
  • Increasing healthy eating 
  • Increasing active living 

LPHUs play a vital role in delivering the Victorian Public Health and Wellbeing Plan 2023 to 2027, which identifies ten priority areas to reduce preventable illness and improve health equity. To support this each of the nine Victorian LPHU develops a Population Health Catchment Plan that aligns with this statewide direction while responding to local need and context. 

Utilising a range of influencing approaches, including a heavy focus on equity, intersectionality and collective impact, our work is often undertaken in partnership and in alignment with other organisations across the catchment. This includes Local Government, Community Health, Aboriginal & Torres Strait Islander Health and Women’s Health sector partners along with many others. 

Through these partnerships, LPHUs drive catchment wide and place-based approaches that are tailored to the needs and priorities of their communities. 

Each year, NEPHU will produce an annual implementation plan to guide in more detail the activity and collaborative opportunities across these priority areas. Through this ongoing, coordinated effort, and with guidance from the Population Health Advisory Group,  NEPHU will continue to strengthen local prevention systems and improve health and wellbeing outcomes for communities across the region.
