This Population Health Annual Report covers the 2024 – 2025 financial year time frame.
NEPHU’s Population Health Annual Report provides a comprehensive overview of the key activities undertaken in the 2024-2025 financial year, often in collaboration with many of our sector partners in the catchment.
Activities reported on include a wide range of health promotion and prevention initiatives, as well as relevant communications, engagement, and events either led by or supported by NEPHU.
Many of these initiatives and activities were delivered over the two-year period from 2023-2025 as part of the NEPHU Population Health Catchment Plan Part 1, focusing on two priority areas for collective action:
- Improving Sexual and Reproductive Health (SRH), and
- Increasing Healthy Eating (HE)
The annual report details how the Plan Part 1 was brought to life and delivered through regional coordination, evidence informed action and collective impact approaches. 16 projects made up the Plan Part 1 along with other place based and catchment wide initiatives that include:
- Project-based initiatives to support capacity building for the prevention workforce, and enhance service delivery in the SRH and HE priority areas.
- The delivery of collaborative grants to support locally driven initiatives with selected priority groups – improving both health and wellbeing outcomes and equity.
- Working and standing with Aboriginal and Torres Strait Islander Communities to co-deliver self-determined and innovative projects.
